APLAR EVALUATION OF BEHCET'S DISEASE DIAGNOSTIC CRITERIA.


Abstract: INTRODUCTION: There are six sets of diagnosis criteria for Behcet's Disease (BD). There is yet no general agreement on which one to use. The International criteria was created to bring this agreement. The aim of this study was to evaluate these criteria, especially the performance of the International criteria. MATERIALS and METHODS: Three countries (China, Iran and Korea) participated in the study. In each country, BD patients were selected as every consecutive patient. Patients from the 3 countries were put together into a single pool. The same was done for control patients with at least one of the major symptoms of BD. The sensitivity, the specificity, and the accuracy of each set of criteria was calculated for the pool of patients. A confidence interval at 95% was calculated for each result. RESULT: The Mason and Barnes criteria was the least sensitive criteria (57.9% ± 6.6). The most sensitive was the Classification Tree (88.4 ± 4.3). The sensitivity of the International criteria was 72.2 ± 6.0. The lowest specificity went to Dilsen criteria (95.9 ± 3.2). The highest specificity went to the Mason and Barnes criteria (100%). The specificity of the International criteria was 99.3 ± 1.3. The least accurate criteria was the Mason and Barnes criteria (78.9 ± 4.2). The most accurate was the Classification Tree (93.2 ± 2.6). The accuracy of the International criteria was 85.8 ± 3.6. CONCLUSION: There is a lack of performance for the International criteria in countries of the APLAR region. A new international study is warranted to improve the performance of the International criteria.
